3 exams/yr. New patient. Recall/Check-up. Emergency, if needed

3 sets of x-rays: Full-mouth x-rays (PAs and BWs).* Check-up x-rays (4 BWs). Emergency x-rays (1 BW and 1 PA)

1 panoramic x-ray* (if needed) or can be partially credited toward a 3D CBCT**

2 routine cleanings (if non-periodontal)

15% off almost all other treatments, including additional exams and periodontal cleanings

15% off almost all treatments for dependents up to age 22

*1 full-mouth x-rays & 1 panoramic x-ray** every 3 years

**In lieu of panoramic x-ray, a $75 credit may be applied toward a 3D Cone Beam CT (includes capture, interpretation, and airway evaluation)
