Understanding Our Fee-for-Service Model
We are an out-of-network provider.
We have chosen this model because we want our relationship to be with you, not with a third party.
This allows us to provide the highest quality of care without the restrictions and limitations imposed by insurance companies. We believe in providing treatment tailored specifically to your needs, not treatment dictated by insurance protocols.
You can still utilize your insurance benefits.
How Reimbursement Works:
Your visit fee is due at the time of service. If you have Out-of-Network benefits, you may be eligible for significant reimbursement.
Our Support to You:
We offer Courtesy Claims Filing Assistance. We will provide you with a comprehensive, ready-to-submit claim form (Superbill) that includes all the information your insurance company needs. You simply mail or upload this form, and your insurer will send reimbursement directly to you.

To find out what coverage you have, call the member services number on the back of your card and ask these three questions:
Do I have Out-of-Network benefits for the following:
Comprehensive exam (code D0150)
Full mouth x-rays (code D0210)
Prophylaxis/routine cleaning (code D1110)
3D Cone Beam CT (this is not likely to be covered, but it doesn’t hurt to ask. Note: We only recommend these as needed)
What is my Out-of-Network deductible and has it been met this year?
What percentage of the fee is covered (your coinsurance)? It may be helpful to view our faq on “How much will the new patient visit cost?” before you call your insurance company.

We cover a lot in the first visit. Plan on setting aside about two hours for this appointment.
We go over: your health history, your concerns, x-rays, collecting and recording data, photos, assessments, review and discussion of findings, and treatment options.
Once you have scheduled an appointment, you will receive a link with our new patient paperwork. It helps to complete it before arriving. If you can’t complete it beforehand, we ask you to arrive 20-30 minutes early.

Most new patients will fall into this category:
Comprehensive Exam - $180
Full Mouth X-rays - $180
If you have a recent (within 6-12 months) set of full-mouth x-rays that are of good diagnostic quality, we may not need to take a full mouth series. This is a case by case basis. We advise you be prepared to take a full series because that is most often the case.
On occasion, we may have time to do a routine cleaning (prophylaxis) on the same day. If you would like a cleaning at the same appointment, please let us know when you are scheduling the appointment and we will try to make it happen.
Prophylaxis - $147

Payment is due at the time of service.
We do not charge credit card service fees.
Payments made with checks can receive a 2.5% discount.
We prefer not to handle cash.
